To receive credit on warranty work that you have performed on Porta-Dock’s products, you must file a warranty claim with Porta-Dock, Inc. We require that you, the dealer, fill out this form completely and upload copies of any paid bills that pertain to this warranty. You must return all defective merchandise to Porta-Dock (unless otherwise notified by Porta-Dock, Inc.), freight prepaid, in order to receive credit on that merchandise. All parts returned for this claim must be tagged with an authorization number and returned with this form. All warranties in excess of $100.00 must be authorized by the factory before work is started.
